Student information

Student Information Sheets are required for the Information Operations School house to properly enroll and graduate students from courses. Failure to submit a form for each class could result in a certificate or DA Form 1059 not being issued.

The Information Operations Battalion or the Vermont Army National Guard does not condone sending Privacy Act Information via e-mail. The preferred method is to fill the form in online, (the form is downloaded to your local PC) print and fax it to the Information Operations Battalion. We have provided two forms for the convenience of our potential students.

Adobe Acrobat (.pdf) forms can be filled in online and printed locally for faxing only. The form data cannot be saved.

MS Word template (.dot) forms can be filled in online and saved to a local PC where they can be attached to an e-mail.  If you will be taking multiple courses, it is suggested to save the filled in form to your local PC. This option should be use at the discretion of the student/user based on the information submitted.
